Preoperative Patient Instructions and Information

Below is information, checklists, and expectations for patients to review prior to surgery. If you have any further questions, please contact your surgeon’s clinic at 920-560-1000.

 

 Surgery Center Education

These documents have been designed by our surgery center nurse optimization team to help prepare you for your upcoming surgery at OSI.

Please review the documents at least 2-3 weeks prior to your surgery. It is recommended that your support person also review this information. This information serves as your pre surgical education.

Orthopedic and Sports Surgery Center is a separate entity from your surgeon’s clinic. We work together to prepare you for surgery.

The items below are surgeon specific topics. Please contact your surgeon’s clinic at 920-560-1000 if you have questions about the following:

  • Driving
  • Return to work
  • FMLA/short term disability paperwork
  • After surgery pain medication questions/refills
  • Handicap parking permit
  • Rescheduling appointments
  • Dental clearance (if required)
  • Additional equipment (ice machine, motion machine)
