Surgery Center Information

This packet was designed by our surgery center nurse navigator team to help prepare you for your upcoming surgery at OSI.

Please review this valuable information prior to your surgery center nurse education call. This call will take place approximately 2 weeks prior to surgery. It is recommended that your support person also review this packet of information.

Orthopedic and Sports Surgery Center is a separate entity from your surgeon’s clinic. We work together to prepare you for surgery.

The items below are surgeon specific topics. Please contact your surgeon’s clinic at 920-560-1000 if you have questions about the following:

• Driving
• Return to work
• FMLA/short term disability paperwork
• After surgery pain medication questions/refills
• Handicap parking permit
• Rescheduling appointments
• Dental clearance (if required)
• CT scan date and location (if having a robotic surgery)
• Additional equipment (ice machine, motion machine)
